You need: Photo ID with current address (must be at least 21), Landlord Contact Info, Vet Contact Info, Proof of Current Vet Records & Vaccines for all pets you have owned in the last 5 years (Dogs: Rabies, Distemper/Parvo, Bordetella, Cats:Rabies &FVRCP) Meet & greets with all residents of your home & dogs required.
